Study title: The Usage of Forecasting in Organizations.

Primary Investigator: Len Tashman, Editor of Foresight: The International Journal of Applied Forecasting

Purpose: The purpose of the survey is to collect information about how organizations develop and use forecasts.

If you chose to participate, you will be asked a short series of questions in a survey format. The survey is multiple choice with options to provide more information on certain questions.

Time required: The entire survey should take no longer than 5 minutes to complete.

Risks and benefits: There are no risks or discomforts anticipated from your participation in this study. There are no direct benefits to you for participating in the study. There is no compensation to you for participating in the study. If you decide to participate, after successful completion of the survey, you will be provided access to a free downloadable copy of an article explaining the survey and the goals of the research.

Confidentiality: No identifying information about you or your firm will be collected, so your responses will be anonymous. Although there is always some risk that online data may be compromised, Crowd Signal (our survey host) uses encryption and other security methods to protect your study information.

Withdrawal from the study: You are free to withdraw your consent and to stop participating in this study at any time without consequence. You can also decline to answer any question you don’t wish to answer. If you withdraw, your information will be discarded.
